Sick notes

To request a sick note:

• To request an extension to a Med3 for up to 2 weeks, please email bobicb-bw.umg@nhs.net with the reason for your request and the dates requested. Please allow 5 working days before chasing.
• Your request will be sent to a GP who will either issue the Med3 or if they need further information, reception will get in touch to advise of next steps.
• Sick notes cannot be requested before the previous one expires so please do not request one before the expiry.
• For new requests or sick notes in excess of 2 weeks, please make an appointment with a GP by calling us on 0118 9874551 at 8am Monday to Friday.  

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
